Indian Registry services are available by appointment Tuesdays through Thursdays. To schedule an appointment, please contact us via email or call 604-824-3266. You can find us at Building 8A – North Side Entrance.
Important information about your appointment
- Ensure that you allow sufficient time for travel
- Late arrivals will be re-scheduled
- No unscheduled walk-ins
If you are applying for the Certificate of Indian Status Card (CIS) (paper laminate), you'll need:
- A (1” X 1”) recent photo of your head and shoulders (No headgear, no sunglasses)
- Two valid IDs, one with a photo
- Same-day issuance
If you are applying for the Secure Certificate of Indian Status Card (SCIS), you'll need:
- Two recent passport photos, one photo must be stamped on the back
- One valid picture ID
- Mail-in process, approximately 17 weeks or more
All registered status members are encouraged to apply for the SCIS card as it is more secure. The card remains valid for 10 years for individuals aged 16 and older, and 5 years for 15 and under.

Please note: One must live on reserve to access services and live on one of the 9 communities CDD Social Services are delegated to provide services to.
The purpose of Adult In-Home Care is to provide support for clients to stay in their home for as long as it is safe and reasonable. The workers provide personal care, meal preparation and light housekeeping.

Stó:lō Elder's Lodge
We offer 15 ground level assisted living suites for seniors and persons with disabilities, each with a kitchenette, bathroom, and living space. Care is provided by a team including nurses, care aids, activity aids, and cooks. Access requires assessment through Fraser Health Home Health. Call 1-855-412-2121 for an assessment.
Elder's Day Program
Join us every Friday for the Elders’ Day Program, a collaboration with Fraser Health that offers seniors enriching activities, a delicious meal, gentle exercises, and valuable socializing time. We gather in Building 7, main floor, and look forward to welcoming you.
Family Caregiver Support
Fraser Health Indigenous Services has launched a new Caregiver pilot project to help families care for elders at home by providing clinical and emotional support, reducing hospital visits, and allowing current elders to stay home longer. The project funding is limited to 20 Elders and at this point is full.

Do check the “For Your Information” list and bring required items.
Earned income, including: wages, rent payments to you, fishing, carving, self-employed income.
Unearned income, including: EI, honorariums, Worker’s Compensation payments, ICBC claims, Bingo or gambling winnings, spousal support and child support.
Residential School reconciliation payments are not seen as income.
Please submit a signed Renewal slip (if applicable, your partner should also sign the form). Additionally, please provide a copy of your monthly active Employment Action Plan. If you receive a notice from your caseworker regarding missing information, please promptly provide the necessary details to avoid any delays in receiving your monthly Income Assistance check. Lastly, please keep your caseworker informed of any changes to your circumstances or income.
You must have an active income assistance file for 12 out of 15 consecutive months.The medical condition must exist for a minimum of 2 years.You will need to obtain the form from your Community Social Development Worker and return it to them once completed by your doctor.
The program will cover the service charge from the doctor, so please ensure that their billing information is completed.
The file is reviewed every 2 years.
Assistance for mobility, such as using a cane, a Seeing Eye dog, or relying on another person for support, is required. This need must have been present for at least two years due to a medical condition. To initiate the process, your Community Social Development worker will need to request the required form. Once you receive the form, have your medical doctor complete their section and fill in your own.
The program will cover the service charge from the doctor, so ensure that their billing information is complete. After completion, return the form to your BSDW for them to fill in their section and send it out for adjudication.
Keep an eye on your mail for the letter informing you of the decision. The file is reviewed annually.
The shelter rate is equal to off reserve income assistance rates. The payment is for actual documented costs for:
- Rent/ mortgage payments
- Hydro
- Heat
- Phone rental
The Monthly Nutritional Supplement allowances are available to PWD classified clients only.
These clients must have a chronic, progressive deterioration of their health, due to severe medical conditions. The extra funds of the Monthly Nutritional Supplement allowance assists with the costs of additional nutritional items that supplement the clients regular diet. In order to be eligible, the client must ensure they have no other funds available to cover these dietary supplements; as well as be on Income Assistance.
Please note that to be eligible for this allowance, a proper application must be filled in by your doctor and returned to your worker for processing. The Monthly Nutritional Supplement program will pay the service charge from your doctor, so please make sure his/her billing information is completed. After returning the filled-in form to your worker, the form is mailed to an adjudicator, accompanied by note from the clients’ doctor.

- A family unit with one adult recipient who has the PWD designation — $16, 200 per year ($ 1350 per month)
- A family unit with two adult recipients where only one recipient has the PWD designation — $19 440 per year ($1 620 per month)

To ensure proper processing of your application, please bring:
FOR CIS Applications:

The processing time for SCIS applications varies. Mail-in applications typically take approximately 17 weeks or more to process. Please plan accordingly and submit your application in advance.
